WebYou probably are trying to designate an up/down staircase on a open space. When you designate it really means "digging" a new stair. You can't dig on open space, you should have designated it on a earth tile. You can then build a downward staircase and replace the ramp with an upward staircase. It's easy to dig out the wrong wall and remove the ramp, leaving the miner stranded. They also like to cause cave-ins when channeling out a section of a ceiling, but that's more my fault.
